Whenever you provide personal information, we are legally obliged to use your information in line with all applicable laws concerning the protection of personal information, including the General Data Protections Regulations 2018. If you have any concerns that your information could have been compromised please get in touch straight away.
What information will Perth Physio Clinic collect about me?
We collect and process information when you telephone to make an enquiry or appointment, when you email us, when you access our online booking system via our website or when you visit the clinic in person. At the point of enquiry or booking, we may ask for your name, date of birth, address, contact numbers and email address. Our telephone answering service is hosted by a third party (Easy Answering) with whom we have a data processing agreement and who are also registered with the Information Commissioner's Office.
When you attend for an appointment, we will collect further information including your medical history and details of your current and previous conditions. We will also ask for information regarding any activities you undertake and your employment. Findings of your physical examination will also be recorded. We have a legal obligation to collect this information; if you do not agree, we will be unable to provide Physiotherapy services.
How will Perth Physio Clinic use the information it collects about me?
We use your information:
- to provide our services and ensure continuity of care;
- to provide a legal record of any treatment or advice we provide;
- to contact you with regards to your ongoing treatment including sending exercises by email. We use a third party for this service (PhysioTec).
Where Perth Physio Clinic proposes using your personal information for any other uses we will ensure that we notify you first. We do not pass your information on for commercial purposes.
When will Perth Physio Clinic contact me?
Perth Physio Clinic may contact you:
- in relation to any appointments you have booked;
- in relation to any correspondence we receive from you or any comment or complaint you make about Perth Physio Clinic;
- to invite you to participate in surveys about Perth Physio Clinic services (participation is always voluntary); and
- for marketing purposes (only where you have agreed to this).
Will Perth Physio Clinic share my personal information with anyone else?
We use electronic records which are hosted by a third party using secure servers (Cliniko). All information is kept electronically in a file that directly relates to your episode of care. Access is password protected and clinical information can only be accessed by practitioners. Records of physiotherapist-led classes are kept on paper, securely stored.
We may pass information with your permission to other medical professionals who may be involved in your care. This may include GPs, consultants, occupational health departments or other Health and Care Professionals. This will either be done by written letter or via password-protected email attachments. Some insurance companies and occupational health referrals require certain information to be shared with them in order to authorise or process your claim. If you do not consent to this, they may not provide cover.
Personal information can also be shared where disclosure is required or permitted by law (for example to law enforcement agencies, including for child protection reasons).
How long will Perth Physio Clinic keep my personal information?
We will hold your personal information for a minimum of 6 years following your last treatment for clients aged 18 and over. For clients under the age of 18, your personal information will be held at a minimum until you turn 25.
Can I delete my data?
Perth Physio Clinic has a legal obligation to retain notes as above. We are unable to delete any data pertaining to your Physiotherapy within this period.
Can I find out what information Perth Physio Clinic holds about me?
You have the right to request a copy of the personal information Perth Physio Clinic holds about you and to have any inaccuracies corrected. We will use reasonable efforts consistent with our legal duty to supply, correct or delete information about you on our files.
What is a cookie?
A cookie is a small file of letters and numbers that we store on your browser or the hard drive of your computer if you agree. Cookies contain information that is transferred to your computer if you agree. Cookies last for different lengths of time depending on what they are used for.

Third party cookies are set by an organisation other than the owner of the website. For example, pages with embedded content from social media and other third party websites such as as YouTube or Facebook may present you with cookies from these websites. These third parties may track your use of the Perth Physio Clinic website.
Where Perth Physio Clinic embeds content from social media and other third party websites, some websites may use Google Analytics to collect data about user behaviour for their own purposes. Perth Physio Clinic Sharing Tools.
You will also see embedded 'share' buttons on the Perth Physio Clinic website. These enable users to easily share content with their friends through a number of popular social networks. When you click on one of these buttons, a cookie may be set by the service you have chosen to share content through. Again, Perth Physio Clinic does not control the dissemination of these cookies.
